WHAT YOU NEED
You need the following to request a US passport:
2. Proof of US citizenship
o Previous passport or certified birth certificate are required for this. See the passport site for details.
3. Proof of identity
o Old passport or photo id (driver's license) are needed for this.
4. 2 passport photos (see number 4 of the passport requirements for photo requirements).
o I was able to get my photo taken where I applied- call you site ahead of time to verify
o It should only cost you $10-15 for the photos.
5. Passport fees(these fees are the same no matter where you go).
o Application fee is $67 and the execution fee is $30 (this is for every adult passport), so the total should be $97 + photo cost.
